1) AVC Retirement Claim Form
Print, sign and return the Claim Form above |
2) Proof of your PPS number
(P60 / P45 / payslip) |
3) Copy of Passport or Drivers License
If your policy is in a different name than on your Passport / Drivers License, please provide both Marriage and Birth Certificate. |
4) Letter from your employer
Outlining the following; Years of service, final salary, Tax Free Lump Sum amount and annual pension amount. |
5) Details of any other pensions in place
Where applicable |
