When you have collected all relevant documents, please submit them through the form below and we will arrange to have your claim processed.
If your AVC is through New Ireland Assurance, print, sign and return the following claim form:
A copy of your P60 / P45 or a recent payslip
If your policy is in a different name than on your Passport / Driver License, please provide both Marriage and Birth certificates.
Outlining the following; years of service, final salary, tax-free lump sum, and annual pension amount.
Digitally sign the below form above to allow us to confirm your pension allowance with your employer.
