Below is a list of the paperwork that will be needed to complete your claim. When you collected all relevant documents, please submit them through the form below and we will arrange to have your claim processed.
1) AVC Retirement Claim FormPrint, sign and return one of the following claim forms, based on which company your pension is invested:
New Ireland – Zurich Life – Aviva
2) Proof of your PPS numberA copy of your P60 / P45 or a recent payslip
3) Copy of Passport or Drivers LicenseIf your policy is in a different name than on your Passport / Driver License, please provide both Marriage and Birth certificates.
4) Retirement Benefit Letter from your employerOutlining the following; years of service, final salary, tax-free lump sum, and annual pension amount.
5) Letter of AuthorityDigitally sign the above form above to allow us to confirm your pension allowance with your employer.

6) Details of any other pensions in place
